The aim is not to hit the tumor with a single high-dose blow, but to build sustainable control by suppressing blood vessel growth, cutting off the tumor’s supply and shifting the immune balance in the patient’s favor. At ONCOMSI, metronomic chemotherapy is considered as part of an integrative treatment roadmap coordinated by the medical oncologist.

On this page you will find what metronomic chemotherapy is, how it works, which drugs are used, which cancer types and which patients it is considered for, its side effect profile and how the treatment process works in Istanbul.

What Is Metronomic Chemotherapy?

In conventional chemotherapy, drugs are given at amounts close to the highest dose the body can tolerate (maximum tolerated dose, MTD), with rest periods in between. These rest periods are needed for fast-dividing healthy tissues such as the bone marrow and digestive system to recover. Metronomic chemotherapy does the opposite: it gives drugs at low doses ranging from roughly one-tenth to one-third of the MTD, without breaks.

The word “metronomic” comes from the steady, continuous beat of the metronome in music. In this approach the drug is usually taken by mouth, daily or weekly, and a low but continuous blood level is maintained without sudden peaks and troughs. Because low doses do less harm to normal cells, treatment can often be continued as an outpatient without hospital admission.

In this respect metronomic chemotherapy is a strategy that questions the cliché “a lower dose means less effect.” The goal is not rapid tumor shrinkage but to wear the tumor down biologically and preserve quality of life while keeping the disease under control. For a closer look at how lowering the dose changes treatment, see low-dose chemotherapy.

Why Was Metronomic Chemotherapy Developed?

High-dose chemotherapy can shrink tumors quickly, but it brings problems such as severe side effects, hospital admissions, mandatory treatment breaks and regrowth of resistant tumor cells during those breaks. In the standard approach, the most resistant tumor cells may have a chance to recover during the long breaks that follow a high dose.

The idea of metronomic chemotherapy arose from the observation that chemotherapy drugs can target not only tumor cells but also the cells lining the inside of blood vessels (endothelial cells) that form the new vessels feeding the tumor. Because endothelial cells divide more slowly than tumor cells, standard intermittent treatments do not affect them enough. When the drug is given at a low dose but without breaks, endothelial cells are not allowed to repair and recover, and vessel growth is kept under constant pressure.

How Does Metronomic Chemotherapy Work?

Metronomic chemotherapy is a multi-targeted treatment based not on a single mechanism but on several complementary effects. The main pathways highlighted in the scientific literature are:

1. Suppressing blood vessel growth (anti-angiogenic effect)

Solid tumors need a new vascular network to grow beyond a certain size. Continuous low-dose chemotherapy selectively suppresses the proliferation of endothelial cells and may reduce the signals that promote vessel formation (for example VEGF). An increase in molecules that block vessel formation, such as thrombospondin-1, and a fall in bone marrow-derived vascular progenitor cells play a role in this process. As a result, the tumor’s oxygen and nutrient supply is restricted.

2. Regulating the immune system (immunomodulation)

Tumors can increase the number of regulatory T cells (Tregs) to suppress the immune system in their favor. Metronomic chemotherapy, especially low-dose cyclophosphamide, can reduce these suppressive Tregs, increasing the activity of the cytotoxic T cells (CD8+) and natural killer cells that fight the tumor. Some chemotherapy drugs may also support the maturation of dendritic cells, which initiate the immune response. This mechanism is the basis for studying metronomic chemotherapy together with immunotherapies.

3. Continuous pressure on resistant tumor cells

Resistant tumor cells can recover during the long breaks of high-dose treatment. Because the metronomic approach applies pressure without breaks, it gives these cells no chance to regrow. In addition, because vascular cells are genetically more stable than tumor cells, resistance to treatment targeting them may develop later.

4. Tumor dormancy and cellular senescence

According to some approaches, metronomic chemotherapy may also trigger processes such as “tumor dormancy” and cellular senescence, which stop tumor cells from dividing. These effects may help the disease remain stable for months.

The Difference Between Metronomic and Standard Chemotherapy

The two approaches differ not only in dose but also in their target and how they are given. The table below summarizes the main differences:

Feature

Standard (MTD) Chemotherapy

Metronomic Chemotherapy

Dose

Close to the maximum tolerated dose

About one-tenth to one-third of the MTD

Frequency

Usually every 1-3 weeks, with breaks

Daily or weekly, uninterrupted

Route

Mostly intravenous

Mostly oral

Main target

Directly killing tumor cells

Blood vessel growth and the tumor microenvironment

Side effects

More marked; supportive care may be needed

Generally milder

Hospital admission

Often needed

Not needed in most cases

An important distinction is that it should not be confused with dose-dense chemotherapy. The dose-dense approach shortens the intervals but keeps the dose high. The metronomic approach lowers the dose and never interrupts it. The two strategies have different biological targets and side effect profiles. For a detailed comparison, see metronomic vs standard chemotherapy.

Which Drugs Are Used in Metronomic Chemotherapy?

Metronomic chemotherapy is not a single protocol but a dosing philosophy. Different drugs can be used alone or together at low, continuous doses. The components most often mentioned in the scientific literature and clinical practice are:

Cyclophosphamide: the most widely used agent, taken by mouth at low daily doses. It has been evaluated in tumors such as ovarian, breast and pancreatic cancer.

Methotrexate: a combination component used with cyclophosphamide at low weekly doses, especially in metastatic breast cancer.

Capecitabine: an oral option that can be given at lower fixed doses over a long period; it requires monitoring for skin toxicity.

Vinorelbine: can be used as several small doses a week, especially in older or frail patients.

Anti-VEGF agents (for example bevacizumab): can be added to metronomic chemotherapy to strengthen vascular suppression.

Immunotherapy (for example pembrolizumab): being studied in combination regimens to support the immune response.

Which drug is used, at what dose and on what schedule varies from center to center and from patient to patient. These decisions are always made by the medical oncologist after assessing the type and stage of the disease and the patient’s general condition. The information on this page is not a prescription. For each drug in detail, see metronomic chemotherapy drugs.

Which Cancers Is Metronomic Chemotherapy Used For?

Metronomic chemotherapy has been studied especially in solid tumors that depend on blood vessel growth. The main clinical areas are:

Ovarian cancer

Recurrent, platinum-resistant ovarian cancer is one of the areas where metronomic chemotherapy was adopted earliest. In these patients, metronomic cyclophosphamide has been evaluated alone or together with anti-VEGF drugs that suppress vessel growth. Combination studies with anti-VEGF reported that the disease could be kept under control without progression for six months or longer in a meaningful proportion of patients.

In addition, studies of a three-drug approach using metronomic cyclophosphamide with anti-VEGF and immunotherapy reported objective response rates in the range of about 40 to 50 percent, with the disease progressing-free for a median of about ten months. Real-world analyses in older and heavily pretreated patients also found clinical benefit rates in a similar range, with treatment generally well tolerated. These results point to the potential of the metronomic approach in ovarian cancer to suppress the disease without harming quality of life.

Metastatic breast cancer

In metastatic breast cancer, the combination of metronomic cyclophosphamide and methotrexate has long been studied. When a drug that suppresses vessel growth (bevacizumab) was added to this regimen, the time the disease spent without progression was reported to lengthen; however, a clear difference in overall survival was not shown in every study. Older studies with metronomic cyclophosphamide and methotrexate found clinical benefit rates of about 30 to 40 percent, meaning the disease could be kept stable for at least six months in a significant proportion of patients.

In breast cancer, the aim of the metronomic approach is less rapid tumor shrinkage than keeping the disease chronically stable and delaying progression while preserving quality of life. It is therefore considered a meaningful option especially for patients whose hormonal options are exhausted and whose bone marrow reserve is fragile, but who should not be left without any treatment.

Pancreatic cancer

Treatment options are limited in advanced or metastatic pancreatic cancer. In selected patients whose disease was controlled with first-line treatment and remained stable for at least a few months, an oral metronomic cyclophosphamide-based maintenance approach was reported to lengthen the time without progression and overall survival, with a low side effect profile.

Although data in this area are still early and not yet mature through large randomized trials, the approach is increasingly discussed in the debate over “should we pause treatment or continue with gentle pressure?” In pancreatic cancer, every decision is made by the medical oncologist according to the patient’s individual situation.

Head and neck cancers, soft tissue sarcomas and some other solid tumors are also areas where the metronomic approach has been studied. Because every patient’s situation is different, whether this treatment is suitable can only be decided after a detailed oncologic assessment. For more on each cancer type, see which cancers metronomic chemotherapy is used for.

Metronomic Chemotherapy and Combination Treatments

Metronomic chemotherapy is often planned not on its own but with other treatments that strengthen its effect. When low-dose cyclophosphamide’s suppression of vessel growth is combined with anti-VEGF drugs cutting off the vascular signal, two-way pressure can be applied to the tumor’s supply bed. This combination has contributed to the time without progression, especially in the ovarian cancer data.

The second important combination is between metronomic chemotherapy and immunotherapy. By reducing the regulatory T cells that suppress immunity, low-dose cyclophosphamide can make the tumor microenvironment more open to immune attack. This may support the effect of immunotherapy drugs that release the immune brake (for example pembrolizumab). The three-drug approach in ovarian cancer, combining a drug that suppresses vessel growth, metronomic cyclophosphamide and immunotherapy, is one example of this synergy in the clinic.

Which combination is used, in what order and in which patient is a complex decision, made only by the medical oncologist after assessing the characteristics of the disease and the patient’s general condition together.

Who Is Metronomic Chemotherapy Suitable For?

Metronomic chemotherapy is not suitable for every patient and does not replace standard treatment. The patient profiles for whom this approach is especially considered in the scientific literature and clinical practice can be summarized as follows:

Patients with limited performance status whose general condition cannot tolerate high-dose treatment

Patients who have received several lines of treatment and whose options have narrowed

Patients with a fragile bone marrow reserve who would struggle to tolerate severe toxicity

Situations in which controlled pressure is the aim and leaving the disease entirely untreated is not desired

Selected patients in whom synergy with immunotherapy is expected

In contrast, in patients with a very rapidly progressing tumor burden that directly threatens organ function, metronomic chemotherapy alone may be insufficient; faster-acting treatments may be needed. The suitability decision always requires an individual assessment.

Side Effects of Metronomic Chemotherapy

Metronomic chemotherapy generally has a milder side effect profile than standard chemotherapy, but this does not mean it has no side effects. Because the doses are low, severe (grade 3-4) side effects are less common. Common effects include:

A fall in blood counts (leukopenia, lymphopenia); severe levels are relatively uncommon

Fatigue, mild nausea and diarrhea; mostly manageable

High blood pressure and protein in the urine with regimens containing anti-VEGF

Skin symptoms such as hand-foot syndrome with capecitabine

With long-term low-dose use, theoretical risks such as cumulative bone marrow suppression and bladder irritation are also taken into account. Regular blood counts and urine tests throughout treatment are therefore important. Most side effects can be managed with dose adjustment, short breaks and supportive treatment. Effects common with standard chemotherapy, such as hair loss and severe nausea, are generally less marked with the metronomic approach.

A low side effect profile is one of the most important advantages of metronomic chemotherapy. It allows treatment to be continued even in older or frail patients. However, “milder” does not mean “risk-free”; the expected side effects and monitoring plan for each patient are explained in detail by the doctor before treatment starts. Any new symptom should be reported to the treatment team without delay. For a full account, see metronomic chemotherapy side effects.

The Place of Metronomic Chemotherapy in Treatment

Metronomic chemotherapy is often seen as “a mild form of classic chemotherapy,” but strategically it serves a different purpose. Rather than rapid tumor shrinkage, it is a sustained-pressure treatment that dries up the vascular bed, loosens immune suppression and wears the tumor down biologically. For this reason, in many cases it is positioned not as a curative method but as a maintenance or bridging strategy that keeps the disease under control.

This positioning means that a high dose is not always necessary. For some patients, continuous but gentle pressure with manageable side effects may be more meaningful for quality of life and time without progression. Especially where leaving the disease entirely untreated is not desired but intensive treatment is not appropriate either, the metronomic approach offers a valuable option.

Is Metronomic Chemotherapy Effective? The Scientific Evidence

The foundations of metronomic chemotherapy rest on pioneering studies showing that low-dose chemotherapy can target blood vessel growth. The study by Hanahan and colleagues published in the Journal of Clinical Investigation in 2000 showed that cytotoxic drugs given on a metronomic (regular, uninterrupted) schedule could target tumor vasculature. In the same year, Klement and colleagues reported that combining continuous low-dose vinblastine with a treatment targeting the VEGF receptor achieved tumor regression without marked toxicity in an experimental model.

Reviews in the following years showed that metronomic chemotherapy is a multi-targeted treatment that not only suppresses vessel growth but also works through additional mechanisms such as immune regulation and tumor dormancy. The 2014 review by Maiti evaluated this multi-targeted effect and the low side effect profile in detail. A recent review published in Theranostics in 2024 summarized how the approach has evolved from salvage treatment in metastatic disease toward maintenance treatment in high-risk patients.

ONCOMSI’s own clinical observations also contribute to this literature. In the 2026 case report by Assoc. Prof. Mehmet Salih İyikesici (İyikesici MS. Curr Oncol. 2026;33(8):489. PMID 42645394), a protocol described by the authors as metronomic in style was used in a patient with drug-resistant advanced pancreatic adenocarcinoma, with cytotoxic drugs given at no more than 50% of their conventional doses and every 14 days. One month later, PET/CT showed a 63.7% reduction in SUVmax (from 18.2 to 6.6), and performance status improved from ECOG PS 2 to PS 0. Treatment-related side effects were limited to Grade 1 nausea, Grade 1 vomiting and thrombocytopenia, with no Grade 2 or higher non-hematologic toxicity. As a single, uncontrolled observation, these findings do not establish causation; the authors state that the results need to be confirmed by prospective, controlled studies.

Although this body of work is encouraging, metronomic chemotherapy is not a curative method on its own in every situation. Much of the research is at an early phase, and studies continue to determine the most suitable drug, dose and timing for each patient. At ONCOMSI, this literature is considered as part of an individual and realistic assessment for each patient; no definite result or guarantee is promised for any treatment.

ONCOMSI’s Approach to Metronomic Chemotherapy

ONCOMSI is an international medical oncology center based in Istanbul, Turkey, and considers metronomic chemotherapy not as a stand-alone intervention but as one component of an integrative treatment roadmap. In this approach, the medical oncologist is the conductor of the treatment: coordinating every step from diagnosis to recovery and deciding how metronomic chemotherapy should be integrated with other methods when needed.

The center’s chief physician, Assoc. Prof. Mehmet Salih İyikesici, is a medical oncologist experienced in metabolically controlled approaches and integrative oncology. Our specialist team, with years of clinical experience, assesses each patient’s situation individually and creates a personalized treatment plan. For more about Assoc. Prof. İyikesici, see Assoc. Prof. Mehmet Salih İyikesici.

At ONCOMSI, when appropriate, metronomic chemotherapy can be planned together with other treatment protocols, above all Metabolically Controlled Oncologic Therapy (MCOT). This integrative view aims for fewer side effects and a more sustainable treatment process.

How Does the Metronomic Chemotherapy Process Work in Istanbul?

For patients who come to ONCOMSI in Istanbul, the metronomic chemotherapy process follows a planned patient journey from start to finish:

Initial assessment and consultation: The patient’s diagnosis, stage, previous treatments and general condition are reviewed in detail. Existing medical records and imaging are assessed.

Personalized plan: The medical oncologist decides whether metronomic chemotherapy is suitable and which drug schedule to choose. The other components of the integrative protocol are planned when needed.

Starting treatment: Drugs are mostly given by mouth in a routine that can be continued at home. In most cases hospital admission is not needed.

Monitoring and follow-up: Treatment response and side effects are tracked with regular blood counts, urine tests and imaging; the dose is adjusted when needed.

This process in Istanbul is planned to be accessible for patients from within the city, from other cities and from abroad. Because metronomic chemotherapy can be given orally and as an outpatient, patients can continue their daily lives as much as possible. For process details for patients from abroad, see metronomic chemotherapy in Turkey and the international patients page.

Arranging the supply of oral drugs and regular blood monitoring is also part of this process.
